• Print Application on 8.5" x 11" paper in black ink on white paper.

  (If you don't have access to a printer, feel free to apply in person at any open application event)

• Completely Fill out application in blue ink only.

  (include all individuals in household add additional add lines if needed)

• Bring Completed form to one of Our Events for processing.

• A current utility bill, student ID, and / or a picture ID is required for each family member.

• Either a one time or 12 Month Client approval will be issued.

• Grant Points for a one time use. Upon approval all Grant Points must be used immediately.

• Grant Points for a 12 month membership approval will be issued monthly, and may be adjusted as

  needed. (A $7.00 Administration fee will be charged to process membership, this fee will be waived for Veterans

  and Individual Members over 55 Years of age, fee may also be waived depending on certain application criteria)

  (All monthly Grant Points must be redeemed in visit)

  Terms and Conditions

• All items are for client personal use only.

• Items sourced for a group. Require an accompanied letter on Groups letter head.

• No items may be sold or bartered.

• All items are distributed in an AS-IS condition with no warrantee or guarantee whatsoever.

• Membership card and ID must be presented at time of Grant Point redemption.

• All persons attending any of Our events assume complete liability to injury or otherwise

• Additional Terms and Conditions may be posted at events

• Any person violating Our T&C will have their membership permanently canceled.
